The Extended Care Facility at APD is a skilled nursing and rehabilitation facility. Many residents stay for a short period of time while recovering for continued recovery from an illness or acute hospitalization.

Residents are encouraged to personalize their rooms and enjoy the assistance and support of our staff while maintaining as much independence in their daily routine as possible. Our top priorities are to maximize the recovery and rehabilitative process for patients who need time between their hospital discharge and their return home. We work to support our patients and residents in their lives while participating in every stage of their care.

Full diagnostic services, social services and rehabilitative services are all conveniently available, and the hospital pharmacy staff takes an active role in care. We encourage you and your family members to visit our facility and spend some time getting to know us.
