All patients seen at our outpatient clinics receive an emailed satisfaction survey, so please be certain to provide your current email address upon registration.
If you do not want to receive satisfaction survey emails, you may unsubscribe at any time. The brief survey may be completed on your desktop computer or your mobile at your convenience. Survey questions include topics such as the ease of making your appointment; waiting times; friendliness; helpfulness; and the quality of your care. Once you complete a survey, you will not be asked to to complete another survey for 180 days.
