NOTICE OF PUBLIC HEARING

PROPOSED AFFILIATION OF ALICE PECK DAY MEMORIAL HOSPITAL

AND DARTMOUTH-HITCHCOCK HEALTH

Pursuant to RSA 7:19-b, notice is hereby given that the Trustees of Alice Peck Day Memorial Hospital will hold a PUBLIC HEARING on September 22, 2015 at 5:30 p.m.in the Dwinnell Room at Harvest Hill, 23 Alice Peck Day Drive, Lebanon, NH

For the purpose of providing information and receiving public comment regarding the proposed affiliation between Alice Peck Day Memorial Hospital and Dartmouth-Hitchcock Health.
