Geriatrics and senior care: a model for kind and comprehensive care
APD is dedicated to providing quality medical care to aging adults. We provide patient- and family-centered care that focuses on the well-being of the aging adult. We can help you develop a comprehensive health care plan with emphasis placed on wellness, preventative medicine and continuity of care.
Our Senior Care program offers home visits through our Geriatrics department for patients who have selected for this service.

If you or a family member is ill, please call as early in the day as possible so that we can assess your needs. We will accommodate you with a same day appointment if at all possible.
Checking in for appointments
Please arrive at least 15 minutes prior to your scheduled appointment time to complete necessary paperwork. If your insurance has a co-pay, it will be collected at this time.
Cancellation and late policy
Please contact the office as soon as possible to cancel a scheduled appointment so that we can offer this appointment to another patient. If you arrive more than 15 minutes after your scheduled appointment time, you may be asked to reschedule your appointment. We will make every effort to have you seen on the same day but this may not be possible.
In order to provide quality care, prescription refills are best addressed at the time of your visit. This allows you to update the provider and to plan any necessary change in your medication or to schedule testing that may need to be done. We do understand that sometimes this is not possible.
Please call your pharmacy directly for routine prescription refills. Routine refill requests are handled Monday through Friday. Please allow 48 hours or 2 business days for processing. Our office will only call you back regarding your prescription renewal if you have completely run out of your medication or you need to see your provider prior to the prescription being renewed.
Most health plans require your primary care provider to refer you to specialists or facilities for certain services within your network. To avoid having your insurance company deny benefits, call our office before obtaining medical care elsewhere unless you believe you have a life threatening emergency. When you need a referral, allow 7 to 10 business days for us to process the necessary paperwork. As always, you should check with your insurance company to determine what your benefits are as referrals do not guarantee payment.

It’s a national problem and a growing challenge in the Upper Valley. Today in the United States, 5% of patients over 65 years old account for 39% of all Medicare spending. Meanwhile, right here in our backyard, nearly 33% of New Hampshire’s entire population will be older than 65 by 2030.
One of the ways APD is addressing this challenge is through its Senior Care program, which addresses the needs of 120 Upper Valley seniors who are at high risk for injuries or ailments that could lead to an expensive admission in a hospital or nursing home. With this aging-in-place model, APD uses an integrated team—including a social worker, care coordinator, and nurse practitioner—to identify highly effective and less expensive ways to make it possible for seniors to enjoy their lives and live at home.
Participants in the Senior Care Program must first become patients in the Geriatric department at APD.
APD Lifecare offers supported senior living options—both assisted and independent. Harvest Hill is an independent and assisted living retirement community located here on the APD campus. Also here at APD, The Woodlands offers an independent living community in spacious, luxurious apartments with assistance by a warm and friendly staff.
